Nomination Form Guidelines
Please read carefully. The selection Committee will consider adherence to these guidelines and procedures when evaluating nominations.
Eligibility:
The Youth Service Award is intended to recognize a young person who has voluntarily contributed time and talent to the betterment of their community. Only students who attend school in the Goose Creek Consolidated Independent School District area, including private and/or home school, ages 14 - 18 will be eligible for nomination.
Nomination Form:
The nomination application (bottom of page) must be completed in full. All nominee and nominator information must be completed either in writing or electronically.
Any individual or group may nominate one or more people and an individual may nominate a relative, himself or herself, as well.
When completing the nomination form, please fill in all blanks and when answering the nomination question please be as concise and thorough as possible. Focus on the nominee’s area(s) of greatest impact and innovation in the community and provide sufficient background information. Attach additional pages, as needed.
The Selection Committee Members will select the award finalist, and verify all information provided.

The nomination deadline is the 15th of every month. Any nominations received after the 15th will be considered for the following month.
