COVID 19, or the Coronavirus

In accordance with the Stay Home, Work Safe order issued by Harris County, the City of Baytown has closed all City facilities to the public until April 30th . Following CDC guidelines, we have canceled all City events, programming, and City venue rentals until further notice.  Some  City services will continue to run, but in a modified format through digital or other distance means. We would like you to know that the City is in daily contact with State and County Health and Emergency Management Officials. We continue to address the situation in cooperation with all of our partners in government and industry. While you’re likely to see various reactions to the situation, we ask that you remain calm.  We will continue to keep you updated here and via our social media channels

How do I obtain a birth certificate by mail?

Complete the Mail In Application for Birth and Death Record Record listing the individual's name, date of birth, father's name and mother's maiden name. This form also includes a Notarized Proof of Identification and an Affidavit of Personal Knowledge.  Scan the completed form and a photocopy of your valid photo ID to vitalstats@baytown.org.  Please note that only applications with a valid Texas Driver’s License and/or ID will be processed by email.  Once the application is verified as complete, staff will contact the applicant on the process for online payment of fees.  Once the applicant completes the online payment, he/she will email the confirmation page to show proof of payment.  Upon receipt of online payment confirmation, staff will mail out the birth certificate via ground mail.  Emergency situations will be handled on a case by case basis.

Present the completed form, a valid picture I.D. and payment to the City Clerk's Office window at Baytown City Hall, 2401 Market Street, Baytown, Texas 77520. The service is available Monday - Friday, 8:00 a.m. to 12:00 p.m. and 1:00 p.m. to 4:30 p.m.  Your request will be processed immediately.

How many copies can I get?

Three copies per transaction

How much does a birth certificate cost?

$23 a copy for either type of form: Certified Copy 5x7 or Certified Copy Long Form.

What if I don't have valid identification?

If you have lost all forms of identification, you may have your parent, sibling, grandparent or a child request the birth certificate for you.

What if I don't know all the information on the application

We cannot provide any information regarding birth or death records; you may research your family information at the following link: www.familysearch.org.

What types of birth certificates are available?

The long form includes all of the information about the child, the hospital, the place of birth and both of the parents. This form is only available for children born within the city limits of Baytown

The short form includes the name of the child, date of birth, parents names, sex, county and state file number.


Who is a qualified applicant

Qualified applicants are the registrant, parents, grandparents, siblings, spouse, or children with proper valid ID as listed below:

  • Parents (valid ID)
  • Grandparents (ID & birth certificate of your child)
  • Siblings (ID & your birth certificate)
  • Spouse (ID & marriage license)
  • Children (ID & your birth certificate)
