Medical Record Request

To facilitate the seamless retrieval of medical and billing records, we have provided the following contact information: 

  1. Attorney Requests: Please submit requests through Chart Swap with Baytown Fire Department specified as the Provider.
  2. Non-Attorney Requests: Can be submitted through any of the following methods:
    • Phone: Call 972-602-2060 ext. 1611 instructions and a medical records request form will be sent  
    • Fax: Secure and direct requests to 800-608-9457
    • Email: Send general information to Emergicon Records, and you will receive a secure email for PHI details
    • Mail: Mail your request to PO Box 180446, Dallas, TX 75218
