Home Elevations

If you are interested in a home elevation for your home there are a few things the city needs from you before you can be considered to be included in a grant application.

These are the documents that you would need to provide to the city (click on the link for samples):

• Home Elevation Questionnaire(opens Word document)
• A Home Elevation Certificate(opens PDF document)
  If you do not have a home elevation certificate you can get one from any local surveyor. The city does not recommend any specific surveying companies. 
• A copy of your current flood insurance declarations page(opens PDF document)
• A copy of your flood loss history(opens PDF document)
  If you do not have a copy of this you can request one using this form(opens Word document).
• Color pictures of all sides of your home(opens PDF document)
• FEMA Declaration and Release(opens PDF document) (Leave the Inspector ID, FEMA Application No., or Disaster No. fields blank)
• Signed and notarized copy of the Model Acknowledgement of Conditions
